THIS IS A LEGALLY BINDING ADOPTION CONTRACT AND
THE THINGS THAT HAVE BEEN AGREED TO CAN AND WILL BE
LEGALLY ENFORCED AT ADOPTERS EXPENSE!
This agreement is made on __________, by and between the
Feline Rescue Group Representative, hereinafter referred to as
“Rescuer” and (Adoptive person’s name)

“adopter(s).
The adopter hereby understands, promises, and agrees that in receipt of this cat to forever release,
discharge and covenant to hold harmless rescuer and by other person, firm, or organization
charged or chargeable with responsibility or liability, their heirs, administrators, executors,
successors, and assigns from any and all claims, damages, and costs, expenses, loss of services,
actions and causes of action belonging to the said adopter, arising out of any act or occurrence
from the present time and particularly on account of the adoption of :

Feline Rescue Group acknowledges receipt of an adoption fee in the amount of
$________________ Check or Cash (Circle One) to aid with reimbursement of veterinary and
other expenses from the adopter. There are additional fees of $_____________ for
___________. There is an extra donation of $______________. There will be a $25.00 charge
for returned checks at which point cash will be required for the reimbursement of the adoption
fee and the returned check fee within 14 days of the
returned check or we will take the animal back.
Yes/No … Adopter is paying $100.00 (cash or check circle one) fully refundable obedience
training deposit, refundable within 5 months of adoption after proof of class completion. Your
check will not be deposited, but held for five months. You can also choose to donate the deposit
once the class has been completed. Feline Rescue Group is currently applying 501c3, and
USDA licensing, and all donations are tax deductible.
Spay/Neuter Agreement
This is a contract of MANDATORY Spay/Neuter as defined herein if the cat/kitten is
not already altered.
This spay/Neuter agreement is an agreement that the female/male cat will not produce a litter of
kittens, either as purposely bred or by accidental breeding. The rescuer’s female/male cat,
adopted as kitten, will be spayed/neutered by 6 months of age if not already done through
Fox Valley Animal League 630-800-3354, 11 John Street North Aurora, Illinois 60542. If a
veterinarian other than Fox Valley Animal League performs the procedure, the adopter is responsible
for and shall
pay all spay/neuter fees and proof of spay/neuter must be sent to Feline Rescue Group by the
time the cat, kitten is 6 months old. If the cat’s health does not allow this agreement to be honored,
a veterinary statement is required. Based on this situation, rescuer, not the adopter, and
the veterinarian will extend the length of time to be spayed/neutered immediately after the cat’s
condition has improved. If this agreement is not honored, the adopter will acknowledge and
agree that the contract has been violated, and the adopter will transport the cat back to rescuer
with no refund. Please understand, we take our adoptions and Spaying/Neutering very seriously,
and feel it is the responsible things to do.
TRANSFER OF OWNERSHIP
The cat shall not be, given to, or adopted to anyone else, nor will there be any transfer of ownership
to any firm, or corporation such as a research facility. If at any time you are unable to
keep the pet, the animal must first be offered back to Feline Rescue Group before other arrangements
are made. Since we are a foster based rescue, space may be an issue. You must
be certain and committed when you adopt an animal. We do
not take our adoptions lightly, and neither should you. We try to place our animals in their forever
homes. No Adoption Fee is refundable after the adoption is complete.
ENVIRONMENT
The cat shall reside indoors, in the adopter’s home, and live as a family member and companion
only. The cat will not be kept outdoors during the adopter’s working hours, or at
any other time left alone outdoors while the adopter is not at home. The cat may be confined
to a room such as a bedroom, or basement, or any other room of adequate size, and
protected from the elements of weather. An outside shelter, such as a doghouse, is not
acceptable as residence. A kennel environment is not acceptable for housing, safety, or
the emotional well being of the cat.
HEALTH PROGRAM
The cat when adopted will be seen by a veterinarian within five (5-7) business days of the date
of the adoption date to be sure the cat is free of parasites, at the adopter’s expense. Adopter
also agrees to continue the routine vaccinations, deworming, annual heartworm testing, and
monthly heart guard. Rescuer reserves the right to check with the veterinarian at anytime to
verify that all of the basic health requirements are being done. Renewal of rabies vaccine is
required by law and therefore by the terms of this contract. The adopter further agrees that any
animal adopted through Feline Rescue Group will not cut/crop tails at any time. You can receive
a free first time office visit for your new pet health exam through Fox Valley Animal League
630-800-3354, 11 John Street North Aurora, Illinois 60542. With any feline rescued through
the Feline Rescue Group.
IMPORTANT NOTICE – TEMPERMENT and HEALTH WAIVER
Rescuer makes no explicit or inexplicit guarantees in reference to the health, temperament,
breed, and size at maturity, appearance of the cat. The cat is adopted “as is” and
the adopter assumes all responsibility for the treatment of any and all existing conditions, or
any other conditions of physical, health or temperament changes that may occur. Rescuer will
provide the cat with routine vaccinations while in our care, at least the first de-worming and any
other tests the rescuer deems fit if the age and current health of the cat permits, and Feline
Rescue Group is not responsible should the cat bite, scratch, or harm any animal, person, or
property in any way once adopted.
While the rescuer makes every effort to place only healthy animals, the rescuer cannot guarantee
the health of any animal, and shall not be held responsible for any medical expenses which
may be incurred or not seen at the time of adoption, and hereby expressly excluding any implied
warranties of merchantability of fitness for the animal is housebroken.
REMEDY FOR NON COMPLIANCE and COST OF ENFORCEMENT
It is agreed that rescuer shall retain superior title in said animal, limited to and for the express
purpose of assuring the animal’s well-being, and only exercise its superior claim in the event it
appears to the rescue that the proper and humane care, as specified in the above adopting
provisions, is not being afforded said animal, in which case the animal may be taken through a
claim and delivery proceeding. The adopter further agrees that if there is any breach or default
of the terms of this contract, that the adopter will pay $500 fine in addition to being held
liable for all court costs and fees for the adopter’s attorney and for rescuer’s attorney. Should it
become necessary for the rescuer to take action to recover an adopted cat, or otherwise enforce
the provisions of this adoption contract agreement, the undersigned adopter(s) will be
responsible for all court costs and all attorney fees representing either the adopter or rescuer.
RESCUER’S RESERVATION OF RIGHTS
Rescuer reserves the right to follow up on the adoption, to inspect the animal and where the
animal is being kept, for the sole purpose of determining compliance with the terms of the contract,
by making a personal visit to the premises of the adopter at any time during the entire
lifespan of the cat.
If the terms and conditions of this contract are not upheld by the adopter, and/or any misrepresentations
have been made by the adopter, rescuer reserves the right to terminate this contract
and the adopters will agree to allow a representative of Feline Rescue Group to reclaim
the cat without notice or refund. The adopter further agrees to pay liquidated damages, in the
amount of $50.00 per day, for every day the adopter fails to comply with agreement terms, or
willingly surrender the cat at time the incident has become knowledge to rescuer.
ADOPTER’S ACKNOWLEDGMENT OF TERMS
1. The felines are not to live in a garage, doghouse, or any type of outbuilding.
2. The felines are not to be left with small children at any time when unattended by
an adult.
3. The felines must have adequate food, water and shelter at all times.
4. The adopter shall refrain from physical abuse of the animal and any inhumane
manner.
5. If for some reason the adoption does not work out, the pet must be offered back to
Feline Rescue Group.
6. No guarantees about the felines temperament, behavior BREED OR WEIGHT AT
MATURITY.
7. All fees are non-refundable.
8. Vet visit within 5 days and Mandatory Spay or Neutering if not already done.
Foster to Adopt Yes_______No_______ If this is a foster to adopt, when the feline is
placed up for adoption you will have first choice to adopting the animal. You have
given Feline Rescue Group an adoption deposit in the amount of
$____________________ which will be deposited or returned on ______________ at
which time you will be responsible for the pet if you choose to adopt, and this contract
including all terms therein are enforceable. You also agree that if you choose
not to adopt, you will keep the animal until a forever home is located. During the foster
period, Feline Rescue Group will pay all medical expenses.
The adopter has read, understands, agrees, and is signing this contract with full understanding of it.
I hereby attest that I have received and reviewed the medical records including the date the animal was obtained, all
inoculations, and any medical treatments done for this animal since the date Feline Rescue Group obtained custody.
